His Prostate and Me

Discouraged with the vacuum device and his continued state of impotence, he and Charlotte waited a very long year to seek other solutions. When Dick was ready to move forward, he visited Dr. Irving Fishman, a leading urologist in Houston who specializes in impotence. First, Dr. Fishman ran a Color Duplex Doppler test and determined that Dick had a venous leak, because the blood would return to the bloodstream before intercourse could be completed. Having a venous leak made it unlikely that Dick could ever sustain an erection on his own again. After this disheartening news, the doctor suggested that he try the tri-mix shots, a mixture of papavarine, prostaglandin and phentolamine, which are injected directly into the penis. While these shots produced a satisfactory erection in about five minutes, it was necessary for Dick to use the highest dosage in order to overcome the venous leak.

Using such a high dose concerned him, because he was beginning to experience residual pain.

When Dick learned that there was a clinical trial underway using MUSE (Medicated Urethral Suppository for Erection), he decided to try it. MUSE is a suppository about the size of a pencil lead, which is composed of one of the same drugs found in the shots. It is placed in the penis with a short wand and creates an erection in ten to fifteen minutes. The first administration of MUSE should be performed under medical supervision. The man should urinate prior to inserting the pellet. The small amount of urine left in the urethra is enough to lubricate the urinary tract for easier application of the MUSE. Other lubricants like Vasoline can interfere with the absorption of the drug. The penis should be pulled upward and outward to straighten the urethra before inserting the wand. This helps minimize or prevent pain for most men.

Dick quickly enrolled in the trial with the thought that a suppository sounded much better than shots. As part of the protocol, he returned to his doctor's office each week, where
